TAMC offers family medicine services for children and adults through its health centers in Presque Isle, Caribou, Fort Fairfield, Mars Hill, and Ashland. We strive to provide compassionate, quality care for the individual health needs of every patient. We believe that providing the best health information we have will enable patients to make the decisions that works best for them and their families. TAMC's health centers are pioneers in accountable care, a new model of healthcare that takes a team approach to caring for you and your family. TAMC and EMHS were one of 31 healthcare systems in the U.S. to be chosen to pilot accountable care, and we continue to make progress toward implementing this model. Accountable care is expected to be of signficant benefit to people who suffer from asthma, diabetes, heart disease, and other chronic disease. TAMC care coordinators, physicians, nurses, and other members of the healthcare team work closely with patients to help those who live with chronic diseases to take control of their health.
